RN Nurse Case Manager- Hybrid

📋 Description

• Lead field staff in delivering client care and clinical services in a hybrid home/field role.
• Provide clinical leadership, coaching, and staff development for pediatric/home care teams.
• Coordinate care plans, documentation, and case reviews with clients, families, and office
• Travel throughout St. Louis County to support safe, high-quality care delivery.
• Balance in-person field visits with home-based administrative work.

🎯 Requirements

• Current MN RN license in good standing.
• Two+ years nursing experience; home health/pediatric experience preferred.
• Experience supervising or mentoring clinicians in home/pediatric settings.
• Trach/ventilator/tube-feeding experience preferred.
• Strong leadership, organization, communication, and problem-solving skills.
• Willingness to relocate to the service area and travel regularly.
