Provide career guidance, case management and follow up to participants to assist them with their
Conduct interviews and assessments to identify participant needs, interests, and abilities.
Ensure participant suitability and eligibility prior to enrollment.
Coordinate with participant to develop Individual Service Strategy/Individual Employment Plan (IEP).
Provide assistance or referrals to participants to successfully complete objectives.
Refer participants to alternate agencies for additional programs for assistance as needed.

🎯 Requirements

Excellent communications skills.
Accurate reporting, timely case noting into system.
Ability to effectively interact with participants.
Knowledge of community social services/support agencies.
Some local travel may be required.
Some occasional nontraditional hours may be required.
