📋 Description

• Act as the post-acute authority and liaison for the Enterprise Practice.
• Serve as executive-level Account Manager with health-system leaders.
• Own post-acute initiatives aligned to Aledade strategy.
• Synthesize claims data, clinical insights, and analytics into workflows.
• Improve LOS and readmission metrics to boost ACO performance.
• Lead quarterly town halls with SNFs, hospitals, and practice leaders.

🎯 Requirements

• Bachelor's degree in a clinical/healthcare discipline or equivalent experience.
• 5+ years clinical experience in a complex healthcare setting.
• 6+ years in value-based care environment (ACO/MCO/large system).
• Autonomy to lead strategic projects and manage exec stakeholders.
• Advanced data-driven collateral to drive change.
• Experience collaborating with post-acute providers.
