Senior Specialty Consultative Pharmacist

📋 Description

• Conduct inbound and outbound telephone interactions with members to discuss medication therapy
• Assess and evaluate members' health conditions with a particular focus on medication therapy and
• Review prescriptions for accuracy, completeness, and appropriateness.
• Identify and evaluate potential drug-drug interactions and other medication-related risks.
• Assess disease state, dosage, directions, and overall therapy to support safe and effective
• Develop and implement comprehensive care plans based on individual patient needs.

🎯 Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy.
• Active, unrestricted pharmacist license from the Board of Pharmacy in Florida, Texas, Ohio, or
• At least 3 years of pharmacist experience in a high-volume retail, hospital, mail-order, or
• Ability to participate in applicable federal prescription programs.
• Comprehensive knowledge of Microsoft Office applications, including Word, Excel, Outlook, and
• Ability to navigate multiple technology systems and platforms and troubleshoot or resolve general
