Social Work and Advocacy Supervisor

📋 Description

• Lead and supervise Social Work, Participant Advocacy, and Behavioral Health teams.
• Ensure person-centered care and align services with participant goals.
• Coordinate enrollment and transitions into the PACE program.
• Partner with IDT and community orgs to support participant needs.
• Foster safety, dignity, belonging, and respect in care.
• Drive continuous improvement and team effectiveness.

🎯 Requirements

• Master’s degree in social work (MSW) or equivalent.
• Must meet state licensure requirements to practice as a Social Worker.
• Minimum 2 years leading, supervising, or overseeing social work or behavioral health teams.
• Strong knowledge of psychosocial, behavioral, and advocacy needs of frail older adults.
• Ability to work with interdisciplinary teams and coordinate care planning.
• Desirable: bilingual in Spanish, Mandarin, or Cantonese.

🎁 Benefits

• Base salary range: $90,000-$108,000 per year.
• Eligible for 10% annual bonus.
