Social Work and Advocacy Supervisor, PACE

📋 Description

• Lead and supervise the Social Work, Participant Advocacy, and Behavioral Health teams, ensuring
• Provide coaching, guidance, and performance management to direct reports to support team
• Oversee participant advocacy and the participant enrollment process, fostering trust, engagement
• Serve as the primary escalation point for complex participant concerns, including grievances
• Partner with the Interdisciplinary Team (IDT) to assess participant needs, support care planning
• Coordinate team staffing, scheduling, and daily work assignments to ensure consistent service

🎯 Requirements

• Master's degree in social work (MSW) or equivalent combination of education, licensure, and
• Minimum of 2 years of experience leading, supervising, or providing oversight to social work, care
• Demonstrated knowledge of the psychosocial, behavioral, and advocacy needs of frail older adults
• Working knowledge of community-based services, government programs, and external resources that
• Ability to prioritize work, exercise sound judgment, and independently manage competing
• Demonstrated ability to collaborate effectively with interdisciplinary teams to support
