Social Work Assistant

📋 Description

• Establish rapport with participants and families for psychosocial support
• Deliver and document social work interventions; arrange resources
• Coordinate care with IDT on housing, DMV, utilities, and more
• Complete IDT care plan reviews with participants and families
• Conduct home, hospital, and skilled nursing visits as needed
• Assist with grievances, appeals, incident reports, and compliance

🎯 Requirements

• High school diploma or equivalency
• Associate degree in social work or related field preferred
• Reliable means of transportation
• Experience assisting people with behavioral health/substance abuse preferred
• Experience solving challenging psychosocial/family situations preferred
• CPR/First Aid within 180 days of hire; certification preferred
