Social Work Assistant

📋 Description

• Establish rapport with participants and families for psychosocial support
• Deliver and document social work interventions and care coordination
• Coordinate services with IDT for housing, DMV, utilities, and other community aid
• Complete IDT care plan reviews with participants and families on schedule
• Conduct home visits, hospital visits, or skilled nursing visits as needed
• Assist with grievances, appeals, incident reports, and APS reports

🎯 Requirements

• High school diploma or equivalency
• Associate degree in social work, human services, social science, or relevant field preferred
• Reliable means of transportation
• Experience assisting people with behavioral health and substance abuse issues preferred
• Experience facilitating solutions with difficult psychosocial/family circumstances preferred
• CPR and First Aid certification within 180 days of hire; certification on hire preferred
