Social Work Assistant

📋 Description

• Establish rapport with participants and families to provide psychosocial support
• Deliver and document social work interventions and arrange resources
• Coordinate housing, DMV needs, utilities, and other community aid with IDT
• Complete IDT care plan reviews with participants and families within regulatory timeframes
• Conduct home, hospital, or skilled nursing visits as needed
• Assist with grievances, appeals, incident reports, and other quality/compliance tasks

🎯 Requirements

• High school diploma or equivalency
• Associate degree in social work or related field preferred
• Reliable means of transportation
• Experience assisting people with behavioral health and substance abuse issues, preferred
• Experience facilitating solutions with difficult psychosocial and family circumstances preferred
• CPR and First Aid certification within 180 days of hire (preferred)
