Lead the Social Work team, overseeing Supervisors, Social Workers, and BH Specialists.
Ensure effectiveness of social work assessments and IDT participation.
Deliver social work interventions per care plans and urgent needs.
Ensure compliance across documentation, assessments, and care plans.
Oversee programming with BH Specialist: support groups, counseling, education.
Conduct home visits as needed.

🎯 Requirements

MSW required.
Social work licensure preferred.
1 year experience with frail/elderly population.
Excellent clinical, organizational, and communication skills with frail elderly.
Valid state driver's license with a clean DMV record.
CPR and First Aid certification within 180 days of hire; preferred on hire.
