Social Work Manager

📋 Description

• Lead Social Work team and oversee social services and behavioral health programs.
• Oversee social work assessments and IDT participation.
• Ensure delivery of social work interventions per care plans and urgent needs.
• Ensure compliance in documentation, assessments, care plans, and annual reviews.
• Oversee programming with Behavioral Health Specialist (groups, counseling).
• Participate in end-of-life care coordination and support.

🎯 Requirements

• Master’s degree in social work (MSW) required
• Social work licensure preferred
• Minimum of one year of experience with frail or elderly population
• Excellent clinical, organizational, and communication skills with frail elderly participants and teams
• Valid state driver’s license with a clean DMV record
• Copy of recent vehicle insurance required
