📋 Description

• Lead Social Work, Advocacy, and Behavioral Health teams.
• Ensure person-centered care in care planning and delivery.
• Align services with PACE standards and regulatory requirements.
• Coach direct reports and collaborate with interdisciplinary teams.
• Oversee participant enrollment and advocacy within the PACE program.
• Drive outcomes and improve participant experience.

🎯 Requirements

• MSW or equivalent; licensure as required by state.
• 2+ years leading social work or care teams.
• Knowledge of psychosocial needs of frail older adults.
• CPR/BLS cert or ability to obtain within 30 days.
• Valid CA driver's license, reliable transportation, auto insurance; travel as needed.
• Strong communication, coaching, and collaboration with IDT.
