Social Worker

📋 Description

• Conduct social work assessments to identify psychosocial needs and goals
• Develop participant care plans in IDT meetings
• Deliver interventions, arrange resources, counsel, and manage cases
• Coordinate with PCP and IDT for smooth care transitions
• Lead care conferences with participants, families, and support networks
• Provide discharge planning when participants disenroll

🎯 Requirements

• MSW required
• Minimum 1 year experience with frail/elderly population preferred
• Experience coordinating care conferences preferred
• Experience assisting with behavioral health or substance use preferred
• CPR and First Aid certification within 180 days of hire
