Social Worker

📋 Description

• Conduct psychosocial assessments and participate in IDT care planning.
• Deliver and document social work interventions and case management.
• Partner with PCP and IDT to guide care transitions.
• Initiate, coordinate, and facilitate care conferences for care coordination.
• Provide discharge planning when participants disenroll from the program.

🎯 Requirements

• Master’s in social work (MSW) required.
• Minimum 1 year experience with frail elderly population preferred.
• Experience coordinating and facilitating care conferences preferred.
• Experience with behavioral health or substance use issues preferred.
• CPR and First Aid certification required within 180 days; maintain thereafter.
