Maintain a caseload of participants and work with the IDT to deliver social services.
Provide psychosocial assessments, care planning, counseling, and case management.
Serve as liaison between participants, families, and care teams.
Support aging safely at home with community resources.
Collaborate with LCSWs on behavioral health issues.

🎯 Requirements

MSW degree required
1+ year frail/elderly population experience preferred
Experience coordinating care conferences preferred
Experience with behavioral health or substance use issues preferred
CPR and First Aid within 180 days; certification preferred on hire
