Social Worker - MSW

📋 Description

• Conduct psychosocial assessments to determine needs, goals, and preferences
• Develop and document participant care plans with the IDT
• Deliver counseling and case management; arrange resources and services
• Partner with the PCP and IDT to guide smooth care transitions between settings
• Initiate and facilitate care conference meetings for care coordination
• Provide discharge planning when participants disenroll from the program

🎯 Requirements

• Master’s degree in social work (MSW) required
• Minimum 1 year experience with frail/elderly population preferred
• Experience coordinating and facilitating care conferences preferred
• Experience assisting people with behavioral health or substance abuse issues preferred
• CPR and First Aid certification within 180 days of hire; maintain thereafter
