Specialty Pharmacy Liaison

📋 Description

• Provide comprehensive medication management to hospital patients.
• Work with clinic team to serve patients face-to-face and by phone.
• Educate patients on hospital services and care outcomes.
• Manage outbound therapy and medication adherence, including refills.
• Communicate with the Patient Service Center about refill reports and appointments.

🎯 Requirements

• State-registered pharmacy technician; CPhT preferred but not required.
• 2 years required; 3-5 years preferred.
• High School Diploma or GED required.
• Strong interpersonal and written/verbal communication; proficient in MS Office.
• Knowledge of transplant, hepatitis C, infectious disease, or oncology a plus.
