📋 Description

• Make home visits to clients in designated territories.
• Evaluate and treat clients with speech, language, or swallowing issues.
• Develop a treatment plan based on evals, orders, and goals.
• Continually assess and revise care plans with the multidisciplinary team.
• Educate patients and families on rehabilitative care and independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• Current VA license as a Speech-Language Pathologist.
• ASHA CCC or equivalent credential.
• Awaiting ASHA exam or certification process.
• Education and supervised experience requirements for certification.
• Solid computer skills; EMR experience preferred.
