Speech Language Pathologist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform evaluations to diagnose and provide speech, language, or swallowing therapy.
• Develop a treatment plan based on evaluation, orders, and goals.
• Continuously assess and revise care plans with the multidisciplinary team.
• Educate patients and families to promote health, safety, independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• Current license as Speech Language Pathologist in STATE.
• CCC-SLP from ASHA, or equivalent.
• Educational requirements evidenced by transcript, pursuing ASHA certification.
• Complete education requirements and pursue supervised experience for certification.
• Strong computer skills; EMR experience preferred.
