Make home visits to clients in designated geographic territories.
Perform evaluations to assess, diagnose, and provide therapy for speech, language, or swallowing
Develop treatment plans based on evaluations and goals.
Revise care plans with multidisciplinary BAYADA team as appropriate.
Educate patients and families on rehabilitative care and activities for health and independence.
Document observations and interventions using a touch pad tablet.

🎯 Requirements

Current PA Speech Language Pathologist license.
CCC-SLP or equivalent certification/experience pathway; ASHA prerequisites acceptable.
Experience with EMR systems preferred.
