📋 Description

• Visit clients in homes across assigned area
• Evaluate and treat speech, language, communication, and swallowing challenges
• Create personalized care plans and adjust as clients progress
• Partner with other clinicians to deliver well-rounded care
• Educate patients and families to support home care
• Document visits and progress using EMR

🎯 Requirements

• Active SLP license in New Hampshire
• Graduate of ASHA‑accredited program
• Comfort with basic technology and documentation systems
