📋 Description

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients
• Develop a treatment plan based on evaluation, physician’s orders, and goals.
• Continually assess and revise the care plan; participate in multidisciplinary team planning.
• Educate patients and families on rehabilitative care and activities to promote health and
• Document observations and interventions using a state-of-the-art touch pad tablet.

🎯 Requirements

• Current license as a Speech Language Pathologist in North Carolina.
• Certificate of Clinical Competence in Speech from ASHA, or equivalent requirements, or pursuing
• Experience meeting certification requirements or accumulating supervised work experience required
• Solid computer skills; prior EMR experience preferred.
