Speech Language Pathologist, (SLP) - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients
• Develop a treatment plan based on evaluation, physician orders, and goals.
• Revise care plans and participate in multidisciplinary team care plans as appropriate.
• Educate patients, families, and care representatives in rehabilitative care and activities for
• Document observations and interventions using an EMR/tablet system.

🎯 Requirements

• Current North Carolina Speech Language Pathologist license.
• Certificate of Clinical Competence in Speech (ASHA CCC) or equivalent qualifications toward
• Completed education/work experience required for licensure and ongoing certification process.
• Solid computer skills; prior EMR experience preferred.
