Speech Language Pathologist, SLP - Home Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech
• Develop a treatment plan based on the evaluation, physician’s orders, and goals.
• Continually assess and revise the speech therapy care plan with the multidisciplinary team.
• Educate patients, family members, or representatives in rehabilitative care for home settings.
• Accurately document observations, interventions, and evaluations using a tablet-based EMR system.

🎯 Requirements

• A current license as a Speech Language Pathologist in Arizona.
• Certificate of Clinical Competence in Speech-Language Pathology from ASHA, or equivalent
• Meet educational and supervised work experience requirements; eligible to take ASHA exam or working
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
