Speech Language Pathologist, Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to diagnose and treat speech, language, or swallowing issues.
• Develop a treatment plan based on evaluation and goals.
• Continually assess and revise the speech therapy care plan with the care team.
• Educate patients and families on rehabilitative care for independence.
• Accurately document observations and interventions in EMR or tablet.

🎯 Requirements

• A current license as a Speech Language Pathologist in Arizona.
• Certificate of clinical competence in Speech (CCC-SLP) preferred or equivalent path toward
• Solid computer skills; prior EMR experience preferred.
• Ability to work in a home health setting and collaborate with a multidisciplinary team.
