Substance Use Disorder (SUD) Case Manager, Contingency Management Program

📋 Description

• Provide health education for CM enrollment; complete intake and orientation.
• Lead CM health/wellness groups, process groups, and drop-ins.
• Coordinate CM activities to promote outreach and positive health outcomes.
• Conduct rapid UA tests; maintain chain of custody; document results.
• Build professional relationships; provide motivational counseling using harm reduction.
• Coordinate care with GLIDE programs and external resources; document progress.

🎯 Requirements

• Strong verbal and written communication; document progress notes.
• Experience with group facilitation and counseling for substance use.
• Crisis intervention and motivational interviewing experience.
• Knowledge of human behavior, development, and cultural patterns.
• Proficiency in MS Office; Salesforce experience helpful.
• Ability to de-escalate difficult clients; maintain professionalism.
