Supportive Housing Case Manager

📋 Description

• Conduct assessments and interviews to determine eligibility.
• Develop and update action plans, case notes, and files.
• Provide ongoing life skills development to improve independence.
• Document care and follow support plans; collaborate with teams.
• Provide targeted interventions when clients risk eviction.
• Connect clients to medical, mental health, and counseling services.

🎯 Requirements

• Resume shows direct housing experience.
• Bachelor's degree in social services plus 1+ year housing coordination.
• High School Diploma or GED plus 2 years housing coordination experience.
• Self-starter with strong time management and problem solving.
• Flexible schedule ability.
• Tech-savvy with MS Office, Zoom, PC, and Android devices.
• Valid driver’s license, insurance, and vehicle access.
• CPR/First Aid certification as needed.
• Lift up to 50 lbs.
• Must be 21 years or older.
• Bilingual preferred.
• Good communication to document care and follow plans.
