Supportive Housing Case Manager

📋 Description

• Conduct assessments and interviews with clients to determine eligibility.
• Develop and update action plans and case notes for a client file.
• Provide life-skills development to help clients improve independence.
• Document care, follow plans, and collaborate with internal and external teams.
• Intervene to prevent eviction when clients are at risk.
• Connect clients to medical, mental health, and other services.

🎯 Requirements

• Bachelor's Degree in social services; 1 year housing coordination experience
• High School Diploma or GED; 2 years housing coordination experience
• Self-starter with strong time management and problem solving
• Ability to work a flexible schedule
• Tech savvy: Microsoft Office, Zoom, Android devices
• Valid driver's license, insurance, and vehicle access
