Supportive Housing Case Manager

📋 Description

• Conduct assessments and interviews to determine eligibility.
• Develop and update action plans, case notes, and client files.
• Provide life skills development to improve independence and quality of life.
• Connect clients with medical, mental health, and other supportive services.
• Guide clients through SSI/SSDI/Medicaid/Medicare/SNAP applications.
• Travel in the area, occasional home visits, and represent Compass at events.

🎯 Requirements

• Bachelor's in social services with 1+ year housing coordination.
• Or HS Diploma/GED with 2+ years related experience.
• Resume must show direct housing experience.
• Self-starter with strong time management and problem solving.
• Flexible schedule; tech-savvy (MS Office, Zoom, PC, Android).
• Valid driver's license and vehicle; CPR/First Aid; 21+; bilingual preferred.
