Supportive Housing Case Manager

📋 Description

• Conduct assessments and interviews with clients to determine eligibility.
• Develop and update action plans, case notes, and other documents related to a client’s file.
• Use reading, writing, and communication skills to document care, follow support plans, and
• Provide ongoing life skills development to assist clients in improving independence and quality of
• Provide specialized and targeted interventions when clients are at risk of eviction.
• Connect clients to medical, mental health, and other supportive counseling services.

🎯 Requirements

• Bachelor's Degree in social services or related field and a minimum of one year experience in the
• High School Diploma or GED and two years of experience in the coordination of supportive housing or
• Self-starter with excellent time management skills and proactive problem-solving abilities.
• Ability to work a flexible schedule.
• Technology savvy, especially with Microsoft Office Applications, Zoom, PC, and Android devices.
• Valid driver’s license, insurance, and access to a vehicle insured for you as a driver.
