Supportive Housing Case Manager

📋 Description

• Conduct assessments and interviews with clients to determine eligibility
• Develop and update action plans, case notes, and other documents related to a client’s file
• Use reading, writing, and communication skills to document care, follow support plans, and
• Provide ongoing life skills development to improve client independence and quality of life
• Provide specialized interventions when clients are at risk of eviction
• Connect clients to medical, mental health, and other supportive counseling services

🎯 Requirements

• Bachelor's Degree in social services or related field and one year experience in coordination of
• Valid driver's license and access to insured vehicle
• Ability to lift up to 50 lbs
• Must be at least 21 years of age
• CPR/First Aid ability
• Self-starter, excellent time management, proactive problem solver
