Telehealth Clinical Resource Nurse

📋 Description

• Monitor patient status remotely using advanced telehealth technology and electronic health record
• Identify changes in patient condition and facilitate appropriate clinical escalation and
• Support bedside clinicians through real-time clinical consultation and workflow guidance.
• Coordinate communication among physicians, nurses, case managers, and other members of the care
• Assist with adherence to evidence-based practices, quality initiatives, and patient safety
• Document clinical activities and recommendations within designated electronic health record systems.

🎯 Requirements

• Active unencumbered Compact RN license
• Active NY license required
• Current BLS and ACLS certifications required
• Minimum five (5) years of acute care nursing experience
• Experience in ICU, step-down, emergency, or progressive care environments strongly preferred
• Previous experience as a Charge Nurse, Preceptor, Team Lead, Clinical Coordinator, or similar
