Telehealth Nurse Practitioner (Heart Failure)

📋 Description

• Virtually assess patients and create care plans per protocols.
• Prescribe and titrate meds per guidelines and standards.
• Optimize guideline-directed therapy for heart failure.
• Respond to nursing alerts about vitals and symptoms.
• Manage changes within APP scope; escalate out-of-scope to physician.
• Coordinate meds, labs, and virtual appointments with the nursing team.

🎯 Requirements

• 3+ years as a Board-Certified Adult/Family NP in chronic cardiology.
• Completed accredited NP program with national certification.
• Active licensure in at least one state: NC, TN, IN, or OH.
• Licensure in 2–3 of GA, KY, PA, VA, TX, FL, CA preferred.
• Willing to obtain additional state licenses as needed.
• Experience managing heart failure patients and therapies.
• Previous telehealth experience strongly preferred.
• Ability to work from home in a HIPAA-compliant environment.
