Telehealth Nurse Practitioner - Heart Failure (Pacific Time Schedule)

📋 Description

• Virtually evaluate patients entering the program and assist in creating a care plan based on
• Respond to alerts from nursing team related to vitals, symptoms, and weight changes.
• Independently manage changes in clinical status within the APP’s scope of practice.
• Escalate out of scope clinical changes to lead MD.
• Liaise with nursing team to order and communicate medications, labs and virtual appointments.
• Partner with the nursing team to reconcile medications on program entry and exit.

🎯 Requirements

• 2+ years as a Board-Certified Nurse Practitioner specializing in heart failure and/or internal
• Active license in California; multiple state licenses strongly preferred.
• Willingness to obtain additional state licenses as needed.
• Fluency in English and Spanish is highly preferred.
• Experience overseeing complex chronic care programs and telehealth strongly preferred.
• Willingness to work in a dynamic, growing company and in a HIPAA compliant home environment.
