Transition Coordinator

📋 Description

• Supervise students in groups and individually; monitor behavior.
• Counsel students on personal, social, behavioral, or academic issues.
• Lead student IEP transition meetings; use assistive tech for voice.
• Manage dream vs. reality dynamics; apply 'failing safely' strategies.
• IDEA compliance and data-driven goal setting for transition plans.
• Track progress with digital tools, rubrics, and self-monitoring.

🎯 Requirements

• Bachelor’s Degree strongly preferred.
• Background in education or social work preferred.
• Regulatory knowledge of IDEA transition mandates.
• Transition & Coaching experience: placing and coaching students.
• Systemic mindset: focus on real-world via community networks.
• Behavioral & performance knowledge: understanding ASD, ADHD, etc.
