Transition Coordinator

📋 Description

• Supervise students in groups/individually; monitor behavior.
• Counsel students on personal, social, behavioral, or academic issues.
• Lead IEP transition meetings; use assistive tech and person-centered tools.
• Balance post-secondary goals with realities; use stepping-stone plans.
• Evaluate student abilities via tests, records, and interviews.
• Develop community sites and work-based opportunities.

🎯 Requirements

• Bachelor's degree strongly preferred.
• Background in education or social work preferred.
• Deep knowledge of IDEA transition mandates.
• Transition coaching and job placement experience.
• Systemic mindset: leverage community networks and transit.
• Behavioral knowledge: ASD/ADHD and student needs.

🎁 Benefits

• Salary range: $45,000 to $75,000 annually.
