Ensure accurate and timely documentation of TOC activities in WelTeam and Athena.
Create authorization shells for TOC services (e.g., transportation).
Coordinate transportation for participant discharges to/from SNFs.
Arrange delivery of DME and medications upon discharge.
Monitor QuickCap and Athena inboxes and respond appropriately.
Collaborate with the TOC Nurse to support care coordination.

🎯 Requirements

High school diploma or GED required.
Medical Assistant certification preferred.
Experience in healthcare, care coordination, or administrative support preferred.
Experience with Transitions of Care and related tasks preferred.
