Transitional Care Manager, Home Health, Nurse, Therapist or Therapy Assistant

📋 Description

• Coordinate initiation of home health care services with discharge planners and families.
• Build relationships with hospital and facility contacts to educate on BAYADA services.
• Manage referral intake and hospital transitions of care.
• Provide clinical expertise to assess home health eligibility and resources.
• Lead a professional BAYADA team in a Burlington, VT territory.
• Engage with referral sources and physicians to grow BAYADA services.

🎯 Requirements

• Current license in good standing as RN, LPN, PT, PTA, OT, OTA, SLP or Social Worker (VT).
• Graduate of accredited nursing/therapy program.
• Strong interpersonal skills; knowledge of Medicare COPs.
• At least 1 year healthcare experience; home health preferred.
• Discharge planning experience a plus; sales/business development a plus.
