Transitional Care Manager, RN

📋 Description

• Coordinate initiation of home health care services with discharge planners, case managers, clients
• Develop relationships with referral sources, physicians and other health care professionals.
• Market BAYADA services by articulating features and benefits.
• Review medical documentation and collaborate with physicians on eligibility for home care services.
• Coordinate with the service office to set up ordered services.
• Maintain weekly referral logs.

🎯 Requirements

• Current license in good standing as RN, LPN, PT, PTA, OT, OTA, SLP or Social Worker in Pennsylvania.
• Graduate of an accredited nursing or therapy program.
• Strong interpersonal skills and knowledge of Medicare COPs and program requirements.
• 至少1 year experience in health care, preferably home health care. (Note: English bullet kept
• Previous discharge planning experience preferred.
