📋 Description

• Take ownership of the quality of care and outcomes for assigned participant panel, working directly
• Conduct PACE required nursing assessments (minimally every 6 months), educate participants and
• Develop and implement a discipline specific plan of care (POC) in collaboration with the IDT
• Coordinate the care for select participants with highest level of complexity, including those in
• Act as the point person for triage, focusing on maximizing WelbeHealth’s internal ability for ER
• Dispense ordered medications to participants while in the center or clinic, and reinforce education

🎯 Requirements

• Graduate of an accredited school of nursing
• Unencumbered RN license
• Minimum of two (2) years of nursing experience in a clinical setting with a frail or elderly
• Highly motivated, self-directed, able to execute tasks in a quickly changing environment and can
• Excellent clinical, organizational, and communication skills in settings with seniors, their
