Complete OASIS Start of Care (SOC) admissions visits for adult/geriatric home health patients.
Conduct comprehensive PT evaluations: mobility, strength, balance, gait, endurance, transfers
Develop individualized treatment plans per patient needs and orders.
Implement interventions to support recovery and independence.
Collaborate with nursing/interdisciplinary team for coordinated care.
Educate patients and families on rehab and safe mobility.

🎯 Requirements

Current New Jersey Physical Therapist license.
Minimum one year licensed PT experience.
Home health experience required.
OASIS experience required.
HCHB experience strongly preferred.
Excellent assessment/communication/time management skills.
