Weekend Admissions Physical Therapist, PT, Senior Living Visits

📋 Description

• Per diem home health visits for senior living communities in designated area.
• Make home visits to clients in designated geographic territories.
• Assess and treat adults/geriatric clients in Independent/Assisted Living.

🎯 Requirements

• A current OH Physical Therapist - PT license.
• Minimum of one year of recent PT experience.
• Graduate from an APTA-approved program or CMAA accreditation program.
• Ability to work independently and manage time; strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Documentation of observations and interventions; client/family education.
