Weekend LPN/LVN

📋 Description

• Deliver direct patient care in accordance with the State Nurse Practice Act
• Implement nursing practices based on comprehensive patient assessments and care plans
• Document patient care accurately and promptly
• Provide patient and family education regarding diagnoses, treatments, and self-care
• Coordinate with hospice services, including aides, OT, PT, MSW, ST, dietitians, and supervisors
• Use equipment and supplies efficiently and effectively

🎯 Requirements

• Graduate of an accredited practical or vocational nursing program
• Minimum of 3 years nursing experience; hospice, community health, or medical/surgical experience
• Current LPN/LVN license in the state
• Committed to professional standards and nursing best practices
• Strong communication, organizational, and interpersonal skills
• Valid driver’s license with insured, reliable vehicle
