Weekend Physical Therapist, (PT) - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements related to mobility, gait, balance, strength, and safety.
• Implement wound care and protective interventions using evidence-based dressings and topical agents.
• Develop and implement individualized care plans with manual therapy, gait training, and balance
• Assess and revise the PT care plan; collaborate with nursing in the multidisciplinary team.

🎯 Requirements

• A current North Carolina Physical Therapist - PT license.
• Minimum of one year recent clinical experience as a Physical Therapist.
• Graduation from an APTA/CAHEA accredited program (as indicated by transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
