1:1 High Acuity Nurse (LPN/RN)

📋 Description

• Provide one-on-one care to clients requiring tracheostomy care and ventilation management
• Deliver private duty nursing in home health care settings
• Administer patient care according to individualized care plans
• Manage high acuity patients' needs, ensuring safety and comfort
• Document patient care electronically using Statewise charting system
• Communicate with clinical support team for guidance and coordination

🎯 Requirements

• Current valid nursing license in the U.S.
• Graduation from a qualified nursing program
• Critical care experience preferred
• Must be comfortable with tracheostomy and ventilator management
• Must be able to work flexible shifts (morning, overnight, 8s, 10s)
• Must be willing to work part-time, PRN, or full-time as assigned
