Complete OASIS SOC admissions visits for home health patients in designated territories.
Conduct PT evaluations: mobility, strength, balance, gait, endurance.
Develop individualized treatment plans based on patient needs and orders.
Collaborate with nursing and interdisciplinary team members to coordinate patient care.
Educate patients, families, and caregivers to promote independence and safety.
Accurately document clinical findings and services using EMR and tablet.

🎯 Requirements

Current New Jersey Physical Therapist (PT) license.
Minimum 1 year of PT experience.
Prior Home Health experience required.
OASIS experience required.
HCHB experience strongly preferred.
Strong clinical, communication, and organizational skills.
