📋 Description

• Complete OASIS Start of Care (SOC) admissions for home health patients.
• Perform PT evaluations of mobility, strength, balance, gait, and safety.
• Develop individualized PT treatment plans based on patient needs and orders.
• Collaborate with nursing and the care team to coordinate services.
• Educate patients, families, and caregivers to promote independence and safety.
• Document clinical findings and services using EMR and tablet tech.

🎯 Requirements

• Current New Jersey Physical Therapist (PT) license.
• Minimum of 1 year of recent PT experience.
• Prior Home Health experience required.
• OASIS experience required.
• HCHB experience strongly preferred.
• Strong clinical, communication, and organizational skills.
