Admissions Physical Therapist

📋 Description

• Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients
• Perform comprehensive physical therapy evaluations, including assessments of mobility, strength
• Develop individualized plans of care based on client needs, goals, functional abilities, and
• Identify additional clinical needs and coordinate appropriate nursing, therapy, social work, and
• Educate clients, families, and caregivers on mobility, safety, fall prevention, therapeutic
• Collaborate with physicians and members of the interdisciplinary care team to coordinate

🎯 Requirements

• Current and unrestricted New Jersey Physical Therapist (PT) license
• Graduate of an accredited Physical Therapy program
• Minimum of one year of recent experience as a Physical Therapist
• Prior home health experience required
• OASIS experience required
• Strong clinical assessment, evaluation, and care coordination skills
