Admissions Physical Therapist

📋 Description

• Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients
• Perform comprehensive physical therapy evaluations, including assessments of mobility, strength
• Assess the client's home environment and identify potential safety concerns, fall risks, and
• Develop individualized plans of care based on client needs, goals, functional abilities, and
• Establish appropriate physical therapy goals and make recommendations for ongoing therapy services.
• Identify additional clinical needs and coordinate appropriate nursing, therapy, social work, and

🎯 Requirements

• Current and unrestricted New Jersey Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Minimum of one year of recent experience as a Physical Therapist.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment, evaluation, and care coordination skills.
