Admissions Physical Therapist

📋 Description

• Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients
• Perform comprehensive physical therapy evaluations (mobility, strength, balance, gait, endurance
• Assess home environment for safety concerns, fall risks, and barriers to independence
• Develop individualized plans of care based on client needs, goals, and physician orders
• Establish physical therapy goals and recommend ongoing therapy services
• Identify additional clinical needs and coordinate with nursing, therapy, social work, and other

🎯 Requirements

• Current and unrestricted New Jersey Physical Therapist (PT) license
• Graduate of accredited Physical Therapy program
• Minimum one year recent experience as Physical Therapist
• Prior home health experience required
• OASIS experience required
• Strong clinical assessment, evaluation, and care coordination skills
